Location: 895 Yard St. (Grandview Yard)

Position Purpose:   

The Mental Health Therapist functions within the Interdisciplinary Treatment Team to provide patient care and supportive services that are directed at achievement of patient care outcomes, maximizing the patient’s involvement in his or her own care plan, improving service levels and decreasing costs. Providing clinical work with patients with mental health diagnoses including individual, group, and couple/family therapy to assist patients to determine treatment goals, return to previous or improved functioning; form aftercare plan; advocate within local community if needed; work as part of clinical team; maintain third party payer relationships.  The Mental Health Therapist provides care to primary care patients experiencing stress or stress-related illness during life events, mood and anxiety disorders. This individual will serve as a critical part of our clinical team, providing psychotherapy and related services for our patients.

Duties and Responsibilities:   

  • Provide effective treatment services to a caseload of clients with mental illness diagnosis on Axis I Diagnostic assessment and diagnosis within the guidelines of the Diagnostic and Statistical Manual of Mental Disorders 5 (DSM- V)
  • Provide psycho-education groups related to general mental health as indicated.
  • Assist client in writing and meeting treatment plan goals and objectives.
  • Work with client to solve problems, consistent with philosophy of the treatment team, and with specific treatment goals of each client.
  • Provide individual, family and group therapy services as indicated.
  • Complete documentation consistent with standards set by the compliance office.
  • Communicate observations of and concerns about clients with team members as needed during the day.
  • Maintain positive contacts with referral sources; consult when needed.
  • Request and provide consultation and/or support as needed from supervisor or team members.
  • Attend and contribute to program staff and treatment team meetings.  Attend staff in-services and other meetings as required.
  • Provide treatment reviews and updates to managed care payers.  Document utilization reviews in the chart so they are easily understood by team members and by the Business Office.
  • Provide clients with links to community resources to help meet their needs.
  • Support community and healthcare linkage as needed for each client (e.g., appointments with outside outpatient providers, psychiatrist, clinical nurse specialist, or primary care physician for medication monitoring and prescribing).
  • May represent the program or hospital by participating in committees, conferences, and public meetings, and by providing education presentations to groups.
  • Other duties assigned within scope of experience, expertise, and credentials
  • Provide excellent clinical services.


  • Master’s Degree in Social Work
  • Independent Ohio license (LISW or LISW-S) to practice 
  • At least two years in independent clinical practice
  • Expertise/training in cognitive-behavioral and/or interpersonal psychotherapy, and understanding of the psychosocial aspects of chronic illness
  • Familiarity with the DSM-V
  • Strong organizational and documentation skills
  • Ability to work independently while also being a strong team player
  • Abide by the Mental Health scope of practice, maintain licensure(s), credentials and CBL’s and practice within the parameters of the Ohio Counselor and Social Work Board
